PROGRAMS

EISOA (Early Intervention Services for Older Adults) Program

County behavioral health funding and collaborative partnership resources through MECCA, with staffing and operational support from Southland Integrated Services. The program goal focuses on older adults, particularly those who are isolated due to social, cultural, and linguistic barriers, are at increased risk of developing mental health conditions such as depression and anxiety. Many older adults experience early onset behavioral health symptoms but remain unserved or underserved due to stigma, lack of awareness of services, limited transportation, and limited culturally appropriate programming. The program has been around since July 2015 and will end on June 30, 2026.

1. Provided outreach and early intervention services to 39 older adults annually.
2. Increased social support participation for 52 individuals through group-based engagement.
3. Improved participant functioning and reduced isolation indicators based on assessments and follow-up.
4. Increased linkage to behavioral health treatment services for older adults at risk.
5. Collaborated with ethnic media to reach more than 21,000 55+ community members to promote behavioral health and preventative care services.

Benefits Enrollment Programs (Medi-Cal, CalFresh, and Public Benefits Navigation)

Many underserved and low-income individuals face barriers to enrolling in health insurance and nutrition assistance programs due to language barriers, limited system literacy, and lack of documentation. Without benefits enrollment, individuals are unable to access preventive care, behavioral health services, or consistent treatment. Program has been serving Orange County residents since November 1, 2024 to current day.

Outcomes

Southland Integrated Services has supported thousands of individuals with benefits enrollment, improving access to healthcare and stabilizing basic needs.
1. Completed 144 Medi-Cal applications/renewals annually.
2. Completed 166 CalFresh applications/enrollments annually.
3. Improved client access to primary care and behavioral health services through insurance coverage.
4. Reduced service delays caused by lack of coverages.

Community Supports Program (CalOptima)

Many low-income Medi-Cal members experience significant barriers to maintaining stable health due to housing insecurity, limited transportation, food insecurity, and difficulty accessing preventive and follow-up care. These unmet social needs contribute to increased emergency department utilization, poor chronic disease outcomes, and inconsistent engagement in behavioral health and primary care services. With Community Supports, we are able to provide housing navigation and sustainability to those who are at risk/facing homelessness.

Southland Integrated Services has provided supportive services and navigation to CalOptima members to reduce barriers to care and improve stability.

1. Served 160 unduplicated CalOptima members annually
2. More than 200 completed care plans and supportive service referrals
3. Provided housing workshops to more than 200 community members about the local housing landscape, budgeting, and voucher programs

Enhanced Care Management (ECM)

High-need individuals with complex medical and behavioral health conditions often experience fragmented care, repeated crisis episodes, and unstable living conditions. Without coordinated care planning, clients struggle to maintain consistent treatment engagement, resulting in avoidable emergency department utilization and worsening health outcomes.

Southland provides care coordination and case management services that support continuity of care, stabilize clients, and reduce fragmentation across systems, all while meeting the patient where they are.
1. Served 427 ECM members annually.
2. More than 250 completed care plans and supportive service referrals

4. Increased linkage to behavioral health, primary care, housing resources, and supportive services
